12/26/2023 EnchantaVaughn Approved 1.Behavior: JaCaeri arrived to the session agitated and refused to communicate appropriately with QMHP-C. JaCaeri had difficulty following the morning routine to put away his things and be seated as directed. JaCaeri was observed displaying a lot active, impulsive, inattentive behaviors by refusing to engage and getting out his seat without permission and asked on several occasions to focus. JaCaeri seemed to be distracted by siblings and phone as evidenced by arguing, fighting and taking each other property. JaCaeri was hitting and kicking his sibling and struggled with focusing on the discussion. 2. Behavior: JaCaeri exhibited frequent angry outbursts that were extreme for the circumstances. JaCaeri appeared to be angry, hostile, and irritable. JaCaeri has recently exhibited an increase in angry outbursts. JaCaeri has started to show significant control of his anger as he does not react as quickly and intensely when angry or frustrated. JaCaeri began to yell and swear. 1.Intervention: QMHP-C explained how behaviors are counter-productive to achieving personal and treatment goals, and offered JaCaeri more positive productive options to behaviors. QMHP-C explained the benefits of completing his work, and encouraged JaCaeri to stay motivated in order to meet specific personal goals. QMHP-C educated JaCaeri on the A-B-C Model to discuss the connection between behavior and consequences. QMHP-C provided JaCaeri to Identify potential triggers to anger and how to intercept behaviors ahead of time. QMHP-C educated JaCaeri on how to use positive self-talk/affirmations instead of resisting the teacher’s requests. QMHP-C educated JaCaeri on using effective communication to communicate needs appropriately. 2. Intervention: QMHP-C assisted JaCaeri with managing feelings of frustration, and discussing building confidence and esteem building. QMHP-C utilized an “anger thermometer” to assist JaCaeri and QMHP-C with identifying JaCaeri anger level and related issues. QMHP-C assigned JaCaeri to draw outline of self and fill picture with objects/images of things to reflect anger. QMHP-C reviewed calming strategies to use when experiencing anger or frustration, such as counting-to-ten, deep breathing exercises, positive visualizations. QMHP-C noted JaCaeri's apparent sad facial affect and processed feelings related to behaviors and overall demeanor. 1.Response: JaCaeri was receptive to QMHP-C's suggestions for today's session. JaCaeri continued to display some active, playful behaviors, but was easier to redirect with intervention. JaCaeri implemented the social skills taught and reinforced in counseling sessions. JaCaeri had difficulty with self-monitoring and reporting his behavioral progress exhibited through the day. JaCaeri refused to participate in the discussion regarding behavioral progress and challenges. 2. Response: JaCaeri responded negatively to QMHP-C talking back and arguing in regards to QMHP-C's feedback based on observations. JaCaeri failed to accept ownership for his behaviors and blamed his peers for his displays. JaCaeri continued to display negative behaviors despite multiple interventions from QMHP-C and guardian contact. JaCaeri continued to display negative, overactive behaviors in the session.
